Worker’s compensation is a no-fault system that protects employees for loss of salary and medical expenses in the event of any injury or illness arising out of and in the course of employment.

Worker’s compensation
Please complete the following steps within 24 to 48 hours of a job-related injury or illness.
- Report the incident to your supervisor immediately. Call 911 if it is a medical emergency.
- University employees working on the Tempe campus can contact ASU Employee Health services for non-emergency injuries, 8 a.m. to 5 a.m, Monday-Friday. All others, Call CorVel at 1-800-685-2877 for non-emergency situations to report the injury to a triage registered nurse. A nurse will do the following procedures:
- Focus on early intervention and your immediate needs
- Connect you with the appropriate level of care.
- Schedule a referral for further intervention.
- Follow up with you the next day to see how you are doing.
- Prescriptions may be filled at any pharmacy, but you must supply the pharmacy with the Arizona Risk Management contact information
- Work with your supervisor to complete the workers' compensation packet. Fax all completed forms to the ASU Benefits Office at 480-993-0007.
- Complete and submit the industrial compensation authorization form
- If medical treatment is sought, ask the treating provider to complete a Health Care Provider Release to Return to Work Certificate of Illness or Injury form. Give the completed form to your direct supervisor so it can be faxed to the ASU Benefits Office at 480-993-0007 before you return to work. You will be sent home if the release to return to work form is not completed by the treating provider.
Failure to report within 24 to 48 hours can result in severe monetary fines, payable by your department. Prompt reporting will accelerate the claim processing and will avoid unnecessary delays or denial of possible benefits and penalties.

Request for physician change
If you voluntarily visit a physician more than once, it is interpreted that you have officially selected your physician. Once you have selected a physician you must request a change in writing through the Industrial Commission of Arizona. It will be at the ICA’s discretion to approve or deny your request.
Supervisors
If you receive notifications of a work-related injury or illness, follow these steps:
- Report the incident online at Employee and non-employee incident report or call 480-965-1823 or 480-727-9669.
- Complete and submit the supervisor’s incident investigation report within 48 hours.
- Report all time missed from work due to the incident to benefits workers’ compensation.
