  • Any other concern for a student’s academic success, safety, or well-being

absence request form

Absence Request Form

We ask that students who are ill and unable to attend classes communicate directly with professors and consult syllabi to understand absence, make-up, and documentation policies.

In an emergency situation (such as prolonged hospitalization and/or times when students are unable to access email) we may notify instructors of these circumstances.

Request an official absence letter from the Office of the Dean of Students by completing the Absence Request Form.

General Information and Resources

  • Absence requests for medical, family, or personal emergencies are reviewed by Student Advocacy and Assistance with appropriate documentation.
  • For absences such as interviews, regular appointments, weddings, personal travel, or other non-emergency leave, please consult your syllabi and communicate directly with your faculty members. Contact your instructors as soon as you become aware of a potential absence via email, office hours, in-person, or their preferred form of communication.
  • Students should review the course syllabus and attendance policies, as the Office of the Dean of Students does not have authority over instructor attendance or grading policies.
