Conduct Checks
For the purposes of employment and educational conduct-related background checks, the Office of Student Rights & Responsibilities can complete background checks for government agencies; undergraduate, graduate, and professional schools; professional and licensure associations; etc. Student authorization is required on all forms.
To request a conduct check, please fill out and sign the Consent Form and email it to the Office of Student Rights and Responsibilities at deanofstudents@asu.edu. Please allow up to 10 business days for processing.
