ASU Health Services Medical Records department maintains an electrical medical record. Your original medical record is property of ASU, but the information in it belongs to you.
As a patient, you are entitled to obtain a copy of your medical record. For the maintenance of your privacy, guidelines are in place to protect your information. Prior to receiving copies of your records or authorizing records to be disclosed you can submit a request via your patient portal or complete an authorization form. If providing the form, you will need a picture ID. You may be charged a copying fee for your records if your records are not going to your patient portal account. You will be notified if any fees will be incurred. To reduce copy charges you may request that recent information or specific documentation be sent directly to yourself. If records are being sent directly from our facility to another facility for continuing care then there is no fee. Processing times for non-medical emergencies can take up to 10 business days.
How do I obtain my medical records?
You can request your records by filling out the needed Medical Records Request Form.
If a student requests to their records released, Health Services is required to obtain a request via patient portal or written release, which will allow the transfer of the student’s medical record. The student can place a request through the Patient Portal, or by completing a Medical Records Request Form. A separate release request is required for each transfer.
How can my parents access my medical records?
After you turn 18, your parents usually no longer have the right to get and amend your medical records just because they are your parent. If you would like your parents to have physical copies of your medical records please fill out the Medical Records Request Form.
If you would like your parents to speak with your doctor please fill out the consent to disclose health care information form.
If you would like to have your parents have access to scheduling treatment, discussing treatment and handling your finances concerning your health treatment please fill out the consent to disclose health care information form.
