DART Registration Process
On-campus transportation is available to students, faculty, staff and guests of the University with permanent or temporary physical disabilities. Rides are provided to and from residence halls, designated parking areas and all campus buildings.
DART (Disability Access and Resources Transportation) Transportation Services are co-sponsored by ASU Parking and Transit Services
Information about DART stops can be found on the ASU Interactive map, you will need to turn on the Accessibility layer and make sure DART Stops is checked as well under the Accessibility Layer.

Complete the DART Services application form. Please double check to ensure all of the information is accurate.

Complete the top portion of the Medical Need Verification form and fill in ONLY the highlighted lines. Provide the form to your treating medical provider to complete the remaining information and have them send it to DART at ASU (Fax: 480.965.0441). Your Medical Need verification form must be completed accurately! If you were treated at ASU Health Services, those locations have our Medical Need Verification form. Ask them to complete the form and fax it to DART.
